As the only New Jersey health insurer to offer plans to residents in every zip code of our state, Horizon understands that each of our members has unique and very personal health challenges. So we are investing in technology and building partnerships that help us better meet, one member at a time, the unique needs of millions of residents in one of the most diverse states in the nation.
For example, consider an elderly woman on a limited income living on a third-floor walkup in Newark. She needs a hip replacement and has no caregiver support. How can the surgery be considered a success if she’s unable to leave her apartment because her building has no elevator? Her needs are clearly very different than those of a single mother living in Atlantic City. Or a college student in Morristown. Understanding the unique challenges –bothclinical and social–of the members and families we serve is critical to improving their health.
We’re working hard to make healthcare local, no matter where our members live or what social and economic barriers they face. To do that, we’re investing in technologies that provide our members with greater access to their personal healthcare data and that help doctors, nurses and hospitals provide more informed and proactive care.
But how do we understand in detail the needs of more than 3.5 million patients? It’s a big challenge. The answer: By unlocking the power of data and by working with innovative companies that help us empower patients and providers.
For example, in partnership with Abacus Insights, we’re unlocking previously unavailable data to deliver better healthcare outcomes and high-value care. In addition, with our investment in AbleTo, which provides virtual mental health care services, our members can tap into a comprehensive national network of providers– including licensed clinical social workers –that offers confidential, personalized virtual therapy.
We’re also focused on providing better care options for New Jersey’s senior population. Braven Health, our Medicare Advantage joint venture with Hackensack Meridian Health and RWJBarnabas Health, is arming physicians with actionable data, including comparative outcomes statistics, that help determine the most effective and affordable treatment options for older patients.
We are also committed to improving the lives of members and reducing costs for those with chronic complex conditions, like diabetes, using data-driven, evidenced based care. Partnering with Aetion, we’re able to analyze data and use pharmacy interventions to identify which type of treatment regimen works best for each member. If a member is not responding well, we can recommend alternative approaches that work better or lower costs.
Together, these partnerships and investments are putting data to work to better track patient outcomes and measure the impact of the services we provide. The goal: Better quality, more informed care that improves the lives of members all across our great state.
